2. Each employee will be contacted privately via phone or e mail and be given instructions on photos that are required.
A phone call or other virtual interview will be completed to gather medical and work history, as appropriate.
3.Review and sign service agreement.
3. Report with recommendations will be completed and sent to employee and supervisor or other referral source.
4. A follow up will be completed approx 2 weeks after report is completed to assess status of recommendations.
4. Credit card payment can be made to Ergonomic Office through this website. Invoice will be provided.
